Actually, it was probably a hallutionation. They said Isaac has mental issues after the 1st game and imagined that happened. He was being driven crazy. and if you pause at 0:11 you can read Isaac returns ALIVE.

we didn’t even get to see him die, and if you really understood the storyline you would know that he was probably hallucinating. think about it, wouldn’t it be really a really bad plot if that was his wife suddenly just happened to be there. anyways, the company did a good thing making an ending like that cause it lets us wonder
